Insurance In-Network
- Aetna
- America’s PPO/HealthEZ
- Anthem BCBS
- BCBS/Blue Cross and Blue Shield
- Magellan Health
- PreferredOne
- UCare
Your health insurance plan may cover services in full or in part. Please check your coverage carefully before the first session by calling your insurance at the number on the back of your insurance card. Here are some helpful questions to ask:
- Do I have mental health benefits?
- Do I have a deductible, and has it been met?
- Do I have a co-pay or co-insurance?
- How many sessions per year does my insurance cover?
- Do I need authorization?
Out-Of-Network Insurance
I am out-of-network with Health Partners, Cigna, and other insurances not listed above. Many insurance programs will reimburse members up to 80% of the fee. Please get in touch with your insurance for the out-of-network benefits option.
Self-Pay
The self-pay option may assure higher privacy and confidentiality. Having insurance pay all or part of your bill for mental health care means that the counselor must give you a mental health diagnosis. This often means that the diagnosis follows you throughout your life and can influence your health and/or life insurance qualification. In addition, insurance may require more details about your presenting problem than you may be comfortable with them knowing. Also, insurance may audit your file, including the case notes.
Sliding-Fee
A Sliding-fee is available on a limited basis. Don’t hesitate to get in touch with me with any questions.
No Suprise Act & Good Faith Estimate
As of January 1st, 2022, Section 2799B-6 of the Public Health Service Act requires all mental health professionals and facilities to provide Good Faith Estimate to clients who are either uninsured or choose not to use their insurance to pay for their therapy services. The No Suprise Act aims to protect clients by educating them about the differences between in- and out-of-network coverage. In essence, you agree to pay a full fee to the provider directly for the services rendered and that the amounts you pay might not count toward your health plan’s deductible or out-of-pocket limit. My services fees are listed above. Please let me know if you require a Good Faith Estimate. For more information, please visit CMS.gov.
Payment
I accept cash, check, credit/debit cards (Visa, MasterCard, Discover, American Express), Health Savings Accounts (HSA), Medical Flex Spending (MFS) accounts, Victim’s Reparations Board, and others. Payment is due at the service date unless other arrangements have been made.
Cancellation Policy
One business day (24-hour) notice is required, except on Monday appointments. Cancellations for Monday appointments are due by 3:00pm on Thursdays. Please note that I am out of the office on Fridays, weekends, and holidays. Because insurance companies do not reimburse me for your failed appointments, I will pass the cost to you. Fees: $70.00 for the first late cancellation; $140.00 for each no-show or late cancellation.
